The most common allergies are caused by everyday, airborne particles such as pollen or house dust mites. For many people, these tiny particles are insignificant. But for people with allergy, they can trigger seasonal or chronic respiratory conditions, such as hay fever or asthma. Allergy significantly reduces people’s quality of life. It affects not only their personal, but also their professional lives. Coping with itching eyes and a blocked nose has its costs for the individual, but also for society in terms of poorer quality of life, lower productivity levels at work and increased sick leave – not to mention the cost of healthcare and medication. While the majority of allergy patients are exclusively offered medication to relieve the symptoms of the allergy, such medication does not treat the allergy itself. And, most patients find that it has no effect, or at best only a moderate effect, on their symptoms. This is the finding of a survey conducted by the European Federation of Allergy and Airways Diseases Patients’ Associations (EFA). Allergy is treatable ALK is committed to improving the quality of life for allergy sufferers by developing products that target the allergy itself, rather than just relieving its symptoms. Allergy vaccination is a well documented therapy, recognized by the World Health Organization as the only truly effective treatment for allergic diseases. The therapeutic principle underlying allergy vaccination is to administer controlled doses of protein substances (allergens) to which the patient is allergic in order to rebuild the immune response tolerance. In this way, the allergic reaction can be reduced and potentially eliminated altogether. Allergy vaccination is traditionally administered as subcutaneous injections or sublingual droplets. But at ALK, our aim is to extend the use of allergy vaccination by introducing convenient tabletbased vaccines – offering more patients the option of a causal allergy treatment. She suffered from eczema (dermatitis), already in her early childhood. One morning, she woke up with difficulties in breathing, swollen eyes and blocked nose, and her doctor concluded that she suffered from grass pollen allergy. Tina started on eye drops, nasal spray and antihistamine tablets, but none of it really helped. During high school, she found out that she also suffered from allergic asthma. At the same time, Tina’s allergies got worse and her eczema had spread to her face, neck, chest and arms. - It was very difficult, especially during spring and summer. When all my friends were outside having the greatest time of their life, I often sat in my room and didn’t want to show myself, says Tina. In the spring of 2002, Tina became a mother, and in the following years her allergies got even worse. - The summer of 2005 was like hell. My son needed to get outside and play, and I was only allowed to take two antihistamine tablets a day. But I often took five, sometimes eight, and it didn’t help, says Tina. After that summer, it was enough for Tina. She went to see her doctor again. The doctor told her that she was going to do a clinical trial with GRAZAX® and that Tina could be part of the trial. Tina saw this chance as her last hope and she volunteered for the study. In March 2006, she started on GRAZAX® treatment. - Already in July the same year, I was able to cut my own lawn, which simply hadn’t been possible before, says Tina. More recently, the focus has been on clinical documentation of the many therapeutic benefits of allergy vaccination. A long series of articles in scientific publications document that ALK’s allergy vaccines significantly reduce allergic symptoms and thereby improve quality of life. This effect is sustained after the completion of treatment. Focusing on the patient’s needs In the 1990’s, ALK was the first company to launch sublingual immunotherapy (allergy vaccination administered as droplets under the tongue). Preventing future allergies Allergic diseases may even come in a series called the ‘allergic march’. A proportion of adults and children with one type of allergy will progress to other allergy types or even asthma later in life. Thus, the development of allergic diseases can be a life-long struggle. Several studies have shown that allergy vaccination reduces the risk of developing additional allergies and asthma. Allergy vaccination remains the only way to stop the allergic march. The World Health Organisation recognises allergy vaccination as the only approach that targets the cause of allergy and alters the natural course of the disease. The benefits that can be gained from allergy vaccination are reflected in the ARIA (Allergic Rhinitis and its Impact on Asthma) treatment guidelines, which recommend early treatment in the disease process in order to prevent further development of allergic rhinitis and/or asthma. Allergic rhinitis has been primarily managed from a symptom relief perspective, i.e. reducing or stopping symptoms with little to no attention on the need to treat the underlying cause of the disease. Despite receiving symptomatic therapies, millions of allergy sufferers experience continual symptoms during the pollen season, limiting their activities and resulting in a poor quality of life. A survey of the burden of allergic rhinitis in Europe showed that despite a diagnosis confirming allergic rhinitis, 83% of subjects felt their condition was a significant health problem because of the burden of symptoms and its impact on general well-being and healthrelated quality of life. In children, symptoms can manifest themselves into learning difficulties. Children with allergic rhinitis may have school problems including absence and poor performance. In addition, sleep disturbance often leads to daytime fatigue and poor concentration in school, resulting in learning impairment. As a supplement to the therapeutic products, ALK manufactures allergen extracts for allergy testing, such as skin-prick tests (Soluprick SQ). Emergency allergy treatment Patients suffering from food allergy or untreated allergy to insect venom may be at risk of experiencing a severe allergic reaction called anaphylaxis, if exposed to the allergen by accident. In such cases the recommended emergency treatment is an intra-muscular injection of adrenaline followed by an immediate call for medical assistance. Adrenaline works directly on the cardiovascular and respiratory systems, reversing the potentially fatal effects by rapidly narrowing the blood vessels to maintain blood pressure, relaxing muscles in the lungs to improve breathing, reversing swelling and stimulating the heart beat. At times, hay fever made David’s life a misery. – I used to dread the summer months, I felt like I had the flu, says David Jenkins. Along with all the usual hay fever symptoms, such as itchy eyes and congested sinuses, I constantly felt lethargic and lacking in energy; like I’d been sedated. My hay fever was sometimes so bad that I had to be sent home from work. As well as trying antihistamines (both tablet and nasal formulations), he even resorted to alternative treatments such as acupuncture. But nothing helped. And over time his symptoms actually got worse. – My hay fever took a turn for the worse five years ago, when the severity of my symptoms increased. The worst experience occurred when I was on holiday in France. I woke up in the middle of the night with the horrible feeling that I was going to die, unable to catch my breath. The only thing I could do to ease the symptoms was to get in the car and turn the air conditioning on to clear my airways of pollen. The following day, David headed straight back home to London and his doctor, who promised David that he would endeavour to find a more efficient treatment than antihistamines. And at the end of 2003, David was able to take part in an allergy vaccine trial. After a skin-prick test, he was diagnosed with an allergy to grass pollen. The treatment needed to be started straight away, outside of the pollen season. After a number of tests, David was prescribed ALK’s allergy vaccine Alutard SQ®. – Now that I am receiving allergy vaccination, most of my hay fever symptoms have disappeared. The severity of the remaining symptoms is about one fifth of what it used to be. During last summer, I only took a total of 10 antihistamines, whereas prior to the treatment, I had to take them every day. Surprised friends and family have also remarked on my transformation during the hay fever season. Because I’m not tired and lethargic, I’m free to be myself – a bubbly, loud and outgoing person. Pharmaceutical form and composition: GRAZAX® are oral lyophilisates (tablets) for specific immunotherapy that contain SQ standardized allergen extracts of Timothy grass pollen (Phleum pratense) at a concentration of 75,000 SQ-T per tablet. Therapeutic indications: Grass pollen induced rhinitis and conjunctivitis in adults with clinically significant symptoms and a positive skin prick test and/or specific IgE test to grass pollen. Posology and method of administration: The daily dose is one tablet to be placed under the tongue. Avoid swallowing for about 1 minute. Start treatment at least 2 months prior to the expected start of the grass pollen season and continue treatment throughout the grass pollen season. First tablet is recommended to be taken under medical supervision (20–30 minutes).Contraindications: Hypersensitivity to excipients, malignancy or systemic diseases affecting the immune system, inflammatory conditions in the oral cavity with severe symptoms, and patients with uncontrolled or severe asthma. Special warnings and precautions for use: Oral surgery, asthma deterioration. Interaction with other medicinal products and other forms of interaction: Concomitant therapy with symptomatic anti-allergic agents (e.g. antihistamines, corticosteroids and mast cell stabilisers) may increase the tolerance level of the patient to immunotherapy. No data on possible risks of simultaneous immunotherapy with other allergens. Pregnancy and lactation: No clinical experience. Animal studies do not indicate increased risk. Treatment should not be initiated during pregnancy. Undesirable effects: Primarily mild to moderate local allergic reactions are to be expected (in studies, 70% of patients). In the majority of patients the reactions start early in therapy, last from minutes to hours and tend to subside spontaneously within 1 to 7 days. If the patient experiences significant adverse events, anti-allergic medication should be considered.
