IKRUUSE Choke Tube Oesophagus irrigation tube This irrigation tube has been developed in co-operation with Dr. Maximillian Pick, veterinary equine specialist, Munich. Indication Obstructed oesophagus (“choke”) Contra-indication Unknown Advantages No aspiration of irrigated fluids or nutrients. No danger of aspiration into the lungs with the risk of inhalation pneumonia. The elastic cuff seals off the oesophagus. To achieve a better sliding effect, the narrow irrigator tube can be sprayed on the outside with SILICONESPRAY (cat. no. 161040). A better sliding effect for the main tube can be achieved by gel-application. Warning In any case of obstruction of the oesophagus there is a risk of aspiration into the lungs (prior to insertion of the oesophagus irrigation tube). Regarding further medical treatment of the above, we refer to the article by M. Pick. In the main tube is a mobile irrigator tube, inserted in a separate lumen, which can be led directly to the obstruction. During irrigation, the irrigator tube can be extended further and further into the impaction. The dissolved mass of forage is hence flushed out through the main tube in a continuous flow. There is no reflux of the masses into the throat area. The oesophagus is only under strain once, i.e. when the main tube is inserted. The mobile irrigator tube, inserted in the separate lumen, can be inserted and extracted several times without irritating the mucous membranes or causing any damage. It is easy to recognize when the impaction is removed: - Back-flow of the irrigation liquids ceases (liquids stay in the stomach) - Markings on the irrigation tube indicate that the tube can now reach all the way into the stomach. Desinfection Use commercially available disinfectants, suitable for medical instruments made of plastic, silicone or latex material. Via the nose, the main tube is inserted into the oesophagus until it reaches the obstruction. The impaction is blocked off from the anterior oesophagus with the cuff. Moving the irrigator tube slowly back and forth, the irrigation of the impaction can progress. Water of body temperature is recommended as irrigation liquid. With caution, the irrigator tube can now be advanced, along the irrigated oesophagus lumen. There is no danger of aspiration into the lungs with the risk of chemical pneumonitis. 1.
